    At NW Bodyworks, I believe that massage therapy is an integral part of a healthy lifestyle. My philosophy is centered around the idea that the mind, body, and spirit are interconnected, and that holistic therapies like massage can help to heal and balance all three.

    I offer a range of massage therapy services, including Swedish massage, deep tissue massage, Lomilomi, medical massage, cupping, myofascial release and more. I will work with you to create a customized treatment plan that addresses your specific needs and goals.

    I am a licensed massage therapist. I completed my massage therapy education at De Anza College in 2003.

    Please print, fill out and bring this form in when you come in for your first massage appointment. If you prefer, you may arrive early for your first appointment and fill the form out in my office.

    REFUND & CANCELLATION PolicY

      

    I understand that sometimes schedule adjustments are necessary and respectfully request at least 24-hour notice for cancellations or rescheduling. If you do not show to your scheduled appointment time, reschedule, or cancel your appointment, cancellation fees will apply according to my policy below:

    • No shows and canceling less than 24 hours prior to your appointment will result in a charge equal to 50% of the service amount when paying out-of-pocket. This does not apply to appointments with insurance coverage.
    • Appointments may be rescheduled at no charge if rescheduled 24 hours or more before your appointment.
    • Two no shows without notice when providing insurance for payment may result in being blocked from future sessions.
    • Please contact me via phone or email to book if you want to use your insurance.


    Please understand that when you forget or cancel your appointment without giving enough notice, I miss the opportunity to fill that opening. Cancellations and rescheduling may be done via text message, email, or on my website. My email address and phone number are in the Contact Us section below. 

    You may book through my phone or email and pay at the time of service as well.
